The application shall contain the name, address and other identifying data of the individual concerned, a description of the request, the preferred form of communication and action under Articles 15-22 of Regulation (EU) 2016/679, signature, date of submission of the application and address for correspondence. In case the application is submitted through a representative, a power of attorney should also be attached.
